NC Driving Record (complete, non-certified)

This can be obtained for a fee at the DMV located at 1100 New Bern Avenue.

You can also obtain a copy online by clicking here. You must have your social security number and NC drivers license number. Then simply download the PDF, print it out, and bring it with you to your assessment. You can also email it to our office at rachel@murray-and-associates.com.

Citation

Your citation, given to you at the time of your arrest, will either be a pink sheet of paper or a printout from the arresting officer. It will contain your information as well as some of the details of your arrest.

If you do not have a copy of this, you can obtain one at the Wake County Clerk of Court's office at 316 Fayetteville Street.

Your attorney may also be able to provide you with a copy. You can also have them fax it to our office at (919) 876-2246 before the time of your assessment.

If your charge is from before 2000, you may not be able to obtain this information. If that is the case, then we are not required to have the citation.

BAC Report

We will need the official breathalyzer reading from the time of your arrest. If you refuse to comply with the breathalyzer test, we will need documentation of your refusal. If you had a blood test done, we will need documentation of that as well.

These are documents that your attorney may have; if so, please have them fax it to our office at (919) 876-2246 before the time of your assessment.

Please note that documentation may be listed on your citation, and will be acceptable if no formal BAC report or blood test can be obtained.

What do I need to bring for my DWI assessment?
These documents are mandatory and are required at the time of the assessment,
unless prior approval is given by a staff member.
Please note that no outcome report will be issued until we have received all necessary documentation.
